As for the scenario: You are visiting your doctor for an ultrasound scan and are required to have a full bladder for that. You are growing restless in the waiting room as the time for your appointment comes and goes, your need to pee growing urgent. Several times, you ask the receptionist how much longer you will have to wait, and are told that the doctor is behind schedule, and that an unanticipated emergency has cropped up, delaying things still further. You struggle to hold on, fidgeting, crossing your legs and holding your crotch. As your desperation mounts, you become impatient with the receptionist, but still she can't get in to see the doctor. Finally, you realize, that you are going to wet your pants and decide to make a dash for the bathroom, but well... it's too late already and you are having a huge accident in the middle of the waiting room. After having wet yourself in front of everybody, you run off to the bathroom and lock yourself in there. Then you call a friend with your cellphone, explaining her the situation and asking her to bring you a pair of dry pants. Then there is a cut and then you are seen changing out of your wet leggings while talking to your friend through the door about your accident and ask her not to tell anybody about it leaving the bathroom. Finally you leave the bathroom.
