As for your situation, you are tied to a pair of spreader bars, one at the ankles which is fixed to the table and anchored quite firmly, and at your wrists. The spreader bat at your wrists has another rope attached to a take-up wheel, and that rope will be slowly pulled to stretch you as if you were on a medieval rack.
