We can see his huge boy belly rounded and swollen from all the food, and probably tinies, sloshing around and being digested in his stomach. To try and ease the pain he rolls over onto his side and the belly hangs down and rests on the couch. It still doesn't seem to help though; the grimaces carry on and so does the discomfort. He hauls himself on all-fours so that his belly can hang down underneath him and gravity can provide more room for digestion.
