Samantha is quite sadistic too, so she continues pushing her large feet over Eleonore's mouth and nose for several minutes, keeping the wet stockings on and having fun to see the expressions of disgust of her poor patient.

Samantha thinks this is not enough for Eleonore, so she slowly takes off the stockings and shows a close up of her bare feet. They are a little dirty and she thinks it is the right condition to continue the "alternative" healing. This time she smothers the patient putting the toes over her nose and closing it tight.
