Derek is a boxer who was supposed to throw a fight for the mob. When he changed his mind in the middle of the fight, the mob came after him, tied him to a wall, and send Vinny to come deal with him.

Vinny is awaiting orders from the boss, so in the meantime, he's taunting Derek with some ballbusting, talking about how they might just take one of his nuts for good...

Eventually Vinny leaves Derek to check on his next orders. On the way out he mentions casually that he'll probably be getting some more boys over with some power tools to castrate Derek.
