His wrists are tied to a tree opposite one of his ankles and his other ankle is tied opposite his genitals and raised so Doug has to hold it up or tug on his sensitive parts. Actually, there is nothing he can move that won't result in a teasing tug. It's not a painful predicament, but he's stuck and there is nothing he can do to get comfortable, or to get off.
