Dr. Fish, however, isn't pleased that Sophie has been having iron injections on such a regular basis, mentioning to Sophie that overdosing on iron can cause various problems; among them, constipation. Nurse Anika calls Dr. Fish back in. The doctor performs a cursory rectal exam and pronounces Sophie in need of a complete exam. Dr. Fish performs a thorough gynecological exam. And, of course, a breast and foot exam. Dr. Fish is thorough. Dr. Fish has Nurse Anika take Sophie's pressure and temperature. Rectally, of course. Sophie is asked for a urine specimen, which she delivers. Dr. Fish then explains what he thinks the problem might be and what tests he's going to do next. First, Dr. Fish administeres an enema, using a large piston syringe. He refills the syringe and does it again. And again. Anika prepares a much larger enema and attaches an inflatable retention nozzle, which Dr. Fish works into Sophie's rectum. Dr. Fish inflates the nozzle, gives Sophie nearly two quarts of water and requires her to hold it for several minutes. The nozzle is left, inflated, in Sophie's rectum so that she won't leak on the way to the restroom, where Dr. Fish releases the air from the inflatable nozzle and Sophie is allowed to pop her cork.
