A psychologist tells the story of new boyfriends Brad and Janet, who find themselves lost and with a flat tire on a cold and rainy end of November 1974. In search of a phone, the couple heads to a nearby castle ("Over at the Frankenstein Place "); there, they discover an ongoing annual Transylvanian Convention, where they meet the Igor-like Riff, his French maid step-sister Magenta, and a groupie named Columbia ("Time Warp"). Dr. Frank-N-Furter, a bisexual and transvestite mad scientist, introduces himself to the couple ("Sweet Transvestite").
In his laboratory, Frank claims to have discovered the "secret of life itself" and gives life to his creation: a tall, muscular and handsome blond named Rocky ("The sword of Damocles"); Frank swears he can turn Rocky into an ideal man in a week ("I Can Make You a Man"). Delivery boy Eddie (half of whose brain Frank used in Rocky's creation) comes out of a freeze on a motorcycle, interrupting Frank, and makes the Transylvanians dance and sing ("Hot Patootie"). When Rocky starts dancing and enjoying the show.
Brad and Janet are shown in separate bedrooms, where each is visited and seduced by Frank, who poses as Brad (when he visits Janet) and then as Janet (when he visits Brad). Although each of them is initially averse to having sex with Frank, each quickly gives in. Janet, distraught and emotional after losing her virginity to Frank, wanders off to find Brad, whom she sees smoking a cigarette in bed with Frank on a video monitor. He then discovers Rocky, huddled in his native tub, hiding from Riff Raff and Magenta, who were tormenting him. While nursing her injuries, Janet, distraught that Brad rested with Frank, decides to get intimate with Rocky as Magenta and Columbia watch from their bedroom monitor ("Touch-a, Touch-a, Touch-a, Touch Me ").
After discovering that Rocky is missing, Frank returns to the lab with Brad and Riff Raff, where Frank discovers that an intruder has entered the building: Dr. Everett Scott, Janet and Brad's former science teacher. Dr. Scott is now investigating UFOs on behalf of the government, which alarms Frank. Dr. Scott explains that he is there looking for his nephew Eddie. Dr. Scott assures Frank that his presence at the castle at the same time as Brad is a coincidence unrelated to UFO work. Frank, Dr. Scott, Brad and Riff Raff then discover Janet and Rocky together, angering Frank and Brad. At this point, Magenta rings the gong to summon everyone to dinner.
Rocky and the guests share an uncomfortable dinner. Janet runs screaming into Rocky's arms, causing Frank to chase her down the halls ("Planet Schmanet Janet"). Janet, Brad, Dr. Scott, Rocky and Columbia all meet in Frank's laboratory, where Frank captures them with the Medusa Transducer, turning them into nude statues ("Planet Hot"). After dressing them up in cabaret costumes, Frank "thaws" them and they perform a live cabaret show, complete with an RKO tower and swimming pool, with Frank as the leader ("Rose Tint My World / Don't Dream It, Be It / What wild and wild ").
Riff Raff and Magenta, now with new space cadet outfits and hairstyles, interrupt the performance. They inform Frank that their mission has failed; Riff Raff declares himself commander and Frank tries to explain himself. Riff Raff and Magenta claim they will return to their Transsexual homeworld in the Transylvanian galaxy, warning Brad, Janet, and Dr. Scott to leave immediately before the castle flies into space. The survivors are then left to crawl in the smog and dirt and the narrator concludes that the human race is equivalent to the insects crawling on the planet's surface: "lost in time and lost in space ... and in meaning".
