Derek is a boxer who was supposed to throw a fight for the mob. When he changed his mind in the middle of the fight, the mob came after him, tied him to a wall, and send Vinny to come deal with him.

Eventually Vinny leaves Derek to check on his next orders. On the way out he mentions casually that he'll probably be getting some more boys over with some power tools to castrate Derek.
Vinny returns to Derek, and he's surprised to say that the mob is going to give him another chance. But before he unties the boxer, he wants to have a little more sadistic fun with his balls while he's tied up...
Now Vinny fucked up big time. He took $50,000 from the mob and lost it on gambling and hookers. So he's tied up on the wall, and it's his turn to get the ballbusting treatment. His balls are going to get worked over by his old partner unless he can come up with the money.
After he reveals that he lost the money the other goon works his balls over some more, the other goon pulls out a pair of plyers and talks about castrating him by crushing his nuts one at a time, then leaves him for what the boss has ordered next...
Surprise Surprise! The Boss has sent Derek, the boxer in trouble, to take care of Vinny's balls! Apparently the mob boss has a sense of justice when it comes to ball busting :)
Derek gloats for a little to Vinny about the tables turning, and then gets to work with some belly punching, then moves the hits slowly down to his cock, then his balls. The gloves come off, Derek's shirt comes off, and it gets heavy with some of Derek's big kicks. (Those big legs have kicked me. They have a lot of weight and momentum behind them, and really drive your balls up into your stomach)
At one point Derek sits down on the floor so he can drive his foot upward into Vinny's balls slow and hard. He actually lifts him completely off the floor.
The boss told Derek that he will be forgiven, if he returns to him with one of Vinny's balls in a jar. He taunts him with plyers, then pulls all the way back and swings them into his balls full . (BTW, this actually really hurt Nito, the actor playing Vinny...)
