In this clip, John Marsh is gagged and tethered to a tree by a rope around his balls. The rope around his balls is prevented from slipping off his genitals by a ring around a chain that is also around his genitals. That chain runs between John's legs and is locked to his belt and the handcuffs securing his wrists. The handcuffs locked to the belt and the chain through his crotch prevents him from reaching around, or reaching anywhere, for that matter.
